Okta sign-in is required.<\/p>\n\n\n\n<p><a href=\"https:\/\/nd.qualtrics.com\/jfe\/form\/SV_0CUIS6GO9udF9rv\" target=\"_blank\" rel=\"noreferrer noopener\">https:\/\/nd.qualtrics.com\/jfe\/form\/SV_0CUIS6GO9udF9rv<\/a><\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Biosafety Signage <\/h2>\n\n\n\n<p>If your lab works under any biosafety level (BSL-1, 2, or 3) you are required to have biohazard signage outside of your lab space with the BSL prominently displayed. In order to get BSL signage, you must reach out to RMS and the Institutional Biosafety Committee and pass an inspection. For more information of biosafety please look at our <strong><a href=\"https:\/\/sites.nd.edu\/lsoprogram\/biosafety\/#BSL\" data-type=\"page\" data-id=\"37\">Sharps and Biosafety Page<\/a><\/strong> or the <a href=\"https:\/\/riskmanagement.nd.edu\/laboratory-safety\/biosafety\/\" data-type=\"link\" data-id=\"https:\/\/riskmanagement.nd.edu\/laboratory-safety\/biosafety\/\" target=\"_blank\" rel=\"noreferrer noopener\">ND Biosafety Manual.<\/a><\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Other Required Documentation <\/h2>\n\n\n\n<h3 class=\"wp-block-heading\">Custodial Services Request Form<\/h3>\n\n\n\n<p>Ensures that building staff can clean your lab space and prevents any accidents or interference with research materials. <\/p>\n\n\n\n<p>Can be found <a href=\"https:\/\/riskmanagement.nd.edu\/assets\/507629\/custodial_services_lab_request_form_v03_1_.pdf\" target=\"_blank\" rel=\"noreferrer noopener\">here<\/a> and looks like this:<\/p>\n\n\n\n<figure class=\"wp-block-image size-full\"><img loading=\"lazy\" decoding=\"async\" width=\"663\" height=\"524\" src=\"https:\/\/sites.nd.edu\/lsoprogram\/files\/2024\/04\/image.png\" alt=\"\" class=\"wp-image-141\" srcset=\"https:\/\/sites.nd.edu\/lsoprogram\/files\/2024\/04\/image.png 663w, https:\/\/sites.nd.edu\/lsoprogram\/files\/2024\/04\/image-300x237.png 300w\" sizes=\"auto, (max-width: 663px) 100vw, 663px\" \/><\/figure>\n\n\n\n<h3 class=\"wp-block-heading\"><strong>Emergency Procedures<\/strong><\/h3>\n\n\n\n<p>All labs need to have an Emergency Action Plan written.&nbsp; RMS has a webpage with Emergency Action Plan templates specific to every building on campus.&nbsp; You can download them from the LSO Drive (Drive\/Resources\/Emergency Action Plan Templates) or from the RMS website (link below).<\/p>\n\n\n\n<p><a href=\"https:\/\/riskmanagement.nd.edu\/general-safety-programs\/emergency-planning-program\/building-department-emergency-actions-plans\" target=\"_blank\" rel=\"noreferrer noopener\">https:\/\/riskmanagement.nd.edu\/general-safety-programs\/emergency-planning-program\/building-department-emergency-actions-plans<\/a><\/p>\n\n\n\n<p>These can serve as a good template to draft (with your PI) an EAP if you do not currently have one.&nbsp; Most parts are already filled out, but you may need to fill in tables such as Critical Operations personnel.<\/p>\n\n\n\n<p>It is a good idea to contact your building manager confirming where the emergency shelter location is for your lab in the event of a tornado. EAPs also benefit from having a map indicating where everyone is to meet during fire alarms (see example below).&nbsp; Consider also having a role sheet to take to the meeting location.&nbsp;<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Standard Operating Procedures (SOPs)<\/h2>\n\n\n\n<p>Each lab should have SOPs for dangerous chemicals (e.g. liquid nitrogen). Furthermore, chemical specific training for each lab is the responsibility of the PI and is required by OSHA. Part of promoting laboratory safety culture includes ensuring SOP documents and chemical specific training are up to date for your lab. While crafting these documents is not a responsibility of the LSO, it is in the best interest of the PI and Lab personnel to generate lab specific safety documentation.&nbsp;<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Training Needs Assessment Form <\/h2>\n\n\n\n<p>Each lab needs to have 1 Training Needs Assessment Form filled out per unique worker type in a lab, i.e. 1 form for graduate students, 1 form for undergraduate students, etc. <\/p>\n\n\n\n<p>The Training Needs Assessment Form can be found <a href=\"https:\/\/docs.google.com\/spreadsheets\/d\/1D5hPuZQSFJYiso2XVcZ-qfX5hEqjD_-6Ru6dWOjSXsk\/edit?usp=sharing\" data-type=\"link\" data-id=\"https:\/\/docs.google.com\/spreadsheets\/d\/1y5NcoocbI2y6378CRfu9rpmK2DTOfM-M\/edit?usp=sharing&amp;ouid=103707155884175720665&amp;rtpof=true&amp;sd=true\" target=\"_blank\" rel=\"noreferrer noopener nofollow\">here.<\/a> This form will need to be copied to be filled out and determines what training needs to be completed by lab members in <a href=\"https:\/\/inside.nd.edu\/task\/all\/endeavor\" data-type=\"link\" data-id=\"https:\/\/inside.nd.edu\/task\/all\/endeavor\" target=\"_blank\" rel=\"noreferrer noopener\">Endeavor<\/a>.<\/p>\n\n\n\n<p>LSOs should apply for dotted line supervisor status <a href=\"https:\/\/riskmanagement.nd.edu\/training\/endeavor\/\" data-type=\"link\" data-id=\"https:\/\/riskmanagement.nd.edu\/training\/endeavor\/\" target=\"_blank\" rel=\"noreferrer noopener\">here<\/a> near the bottom of the page.<\/p>\n\n\n\n<p><\/p>\n","protected":false},"excerpt":{"rendered":"<p>Contained in this section: Required Signage Posted Outside of Lab Spaces Below are the documentation labs need to have posted at the front door of the lab.
