• Links will take you to either a Stata (.dta) data file or a .csv data file.
• Please complete this short form to inform us that you have downloaded data and are using it for research purposes.  We will create a Google Group of data users that will allow us to inform you of updated data releases and documentation.
• Please inform us via email (nethealth@nd.edu) of any publications and reports generated from using the data.  We would like to keep track of the people working with the data and the resulting research.
